Medical Records
To request medical records, please complete the PDF form and email it to info@redhookfamilypractice.com.
Providers and medical facilities may fax requests to 855-279-4420 or email info@redhookfamilypractice.com. Please ensure that all requests include the patient’s signature authorizing release of records, as required by HIPAA.
Registration Form
Whether you are a new patient or an existing one, by completing the form prior to your arrival for your appointment you save time and your documents are automatically sent to Red Hook Family Practice.

From this website you can fill out a new patient registration form and if you currently are a patient you can log into their portal and update your information.
Alternatively, you may click the icon, complete the PDF registration form, and return it to our office or email it to rhfamilypractice@redhookfamilypractice.com.
Employers Interested in Working with Us
Red Hook Family Practice PC works with employers and third-party organizations to provide physical exams and onboarding-related medical services for employees.
We also offer Accredited Drug Testing for individuals who require testing for work. Red Hook Family Practice is a registered Medical Review Officer (MRO) site and provides urine drug screening for pre-employment, random testing, post-accident testing, Coast Guard requirements including captain licenses, and self-referrals.
Employers interested in partnering with us may email patientcare@redhookfamilypractice.com. We would be happy to discuss your needs and work on a contract that best supports your organization.
Prescription Refills
To request a prescription refill, please log into your portal with your updated medication information and preferred pharmacy.
Please allow up to 3 business days for your request to be reviewed and processed. Refill requests are reviewed by our clinical team to ensure the medication is appropriate and safe to continue.
If you are out of medication, need an urgent refill, or are requesting a controlled substance, please schedule an appointment with our office.
Weight Loss Program
Interested in starting our Weight Loss Program? Please email our Nurse Administrator for more information.
Our team member is experienced in weight loss care and will review your inquiry to discuss available options with you in more detail.
Please note that there is currently a waitlist for the program. Patients can expect to receive a response within 3 business days.
Not everyone qualifies for the program. This review process helps us determine whether the program is safe and appropriate for you, so we can provide the best possible care.
